This is kind of a little thing, but the 2nd favorite gift I ever got from ratbastid was a telescoping backscratcher. That little guy goes with me everywhere. I'm one of those weirdos who can't reach the middle of my back, and I always seem to have an itch.
This is edged out only slightly by my new iPod (60GB Video! w00t!)
In terms of gifts I have given to my "umfriend" D, I installed a walk-in closet and painted and re-decorated her bedroom while she was gone for holiday break last year. Hard to beat that.
