For me it's the whole scenario. The actions and reactions of the couple to each other. The sound is a turn on if it's real. If it's over the top and seems like acting, it's a turn off. I think one of the most important things for me is that it's got to seem real. I've got to be able to envision myself in a similar scenario, feeling the same things.
