Like Daniel, I've seen reverse up next to first and down 'below' fifth.
Usually, if reverse is 'up', you also have to push the gear lever down (towards the ground) to engage reverse - it makes it almost impossible to accidentally engage reverse.
Reverse next to first was quite common in older 4 speed gearboxes.
