The worst game that I've played, hands down, was Quest for the N64. It was the first (afaik) RPG-style game for the platform released in North America, so I scooped it up. I think it was a rush job, because the controls were horrible. Not to mention, after a battle, you'd get turned around, and then lost your bearings. I spent 2 hours going around in circles in some cave before I gave up.
Sadly though, the game had a lot of potential. A lot of the graphics were well-designed, the music was good, and the storyline, if developed further, would have been great.
